I added the authenticator in both 1password on my Windows PC, and Microsoft Authenticator on the iPhone, so I don't think the clock could explain it, unless Square Enix's servers are expecting a different time zone.
You use both your regular password and a six-digit code (that's different every time) that the app gives you. In theory, those six-digit codes are only useful for a minute or so, and it's very hard for a hacker to figure out what they would be unless they have your smartphone.
