If you are getting public benefits, are a low-income person, or do not have enough income to pay for your household’s basic needs and your court fees, you may use this form to ask the court to waive your court fees. The court may order you to answer questions about your finances.
Fill out the Request to Waive Court Fees (form FW-001). This is a confidential form that only the court will see. On item 5, check a, b, or c depending on why you qualify. If you checked a (you qualify because you receive public benefits), you do not need to complete Page 2.
