New and transfer students are required to complete their health forms and immunization requirements through the Student Health Portal. You can access the Student Health Portal using your Emerson username and password and will find other frequently used forms on this site.
Emerson Wellness Center offers in person and telehealth appointments for both Counseling and Health Services. Please call to schedule an appointment at 617-824-8666. Any students seeking care will be scheduled for the next available appointment.
Log into the Student Health Portal at https://emerson.medicatconnect.com/ and click on “Medical Forms.” There are three (3) forms: Incoming Student Health History Form, TB Risk Assessment Questionnaire (to be completed AFTER JUNE 1), and a Telehealth Agreement.
23 gru 2021 · Students must upload documentation of their COVID-19 vaccine to the Student Health Portal AND enter the date of their booster as soon as possible. The Center for Health and Wellness will review all documentation.
Immunization requirements are listed on the Immunization Verification form and must be submitted through the Student Health Portal. All required immunizations are available at Health Services for eligible Undergraduate and Graduate Students.
