Verification to Act as a Supervisor for Speech-Language Pathology Professional Experience. Applicant MUST have an identified PA licensed supervisor at the time of application. Download Supervision Verification form and provide the form to your supervisor. Supervisor complete the bottom portion.

Applicants for a license as a Speech-Language Pathologist must possess a master's degree or higher in speech-language pathology from an academic program accredited by an accrediting agency approved by the State Board of Examiners in Speech Language Pathologist & Audiologists.
