You will be paid if your employer pays employees on temporary disability, if you use accrued paid time off (such as vacation time), or if you apply to the California Employment Development Department (EDD) for State Disability Insurance (SDI) and qualify.

Complete and sign Part A on page 1 and Part B on page 3 of the Claim for Paid Family Leave (PFL) Benefits, DE 2501F, and mail no later than 41 days from the date you wish to begin your bonding claim. Attach documented evidence of your relationship to the child with whom you wish to bond.
