File a Bonding Claim. Mothers Transitioning from a Disability Insurance (DI) – Pregnancy-Related Claim to PFL. When you have recovered from delivering your baby and after your final DI payment is issued, we will send you a Claim for Paid Family Leave (PFL) Benefits – New Mother (DE 2501FP).
You will be paid if your employer pays employees on temporary disability, if you use accrued paid time off (such as vacation time), or if you apply to the California Employment Development Department (EDD) for State Disability Insurance (SDI) and qualify.

By my signature on this bonding certification, I authorize the medical provider, adoption agency, adoption party(ies), or foster care placement agency to disclose to the Employment Development Department all facts concerning the birth, adoption, or foster care placement of the above-named child.
