Article Info. Download PDF. Abstract. Renal impairment is common worldwide and poses a significant challenge to safe drug prescribing. As renal function falls, drug clearance decreases and exposure to free drug often increases, leading to increased risks of toxicity and adverse drug reactions.

On the other hand, unnecessary decreases in dosage may result in undertreatment, or changing to an alternate drug with a narrower therapeutic index, lower efficacy or both.

Drug dosage adjustment for patients with acute or chronic kidney disease is an accepted standard of practice. The challenge is how to accurately estimate a patient’s kidney function in both acute and chronic kidney disease and determine the influence of renal replacement therapies on drug disposition.
