You can only rip DVD to MKV with MakeMKV. You can re-convert MKV to MP4 with Handbrake. To rip DVD to ISO, you can try WinXDVD or DVDFab.
When I rip a DVD I usually get rid of all the extra "folders" that MakeMKV finds when I open the disk. I only select the biggest one which is usually the full movie with subtitles and audio options, leaving the menus and deleted scenes behind.
There are two options with MakeMKV. Rip to ISO, which gives you an exact image of the DVD or Blu-Ray including menus or rip and remux each video into an MKV container. In either case, as stated above you'll retain the full size quality of the original video. A DVD ISO can be easily played with most software and hardware media players.
