I made this template for my own use, but figured I'd circulate it in case someone out there needs something like this. Each sheet can print of 8 playing-card-sized item cards. The layout for this card started as simulation of the fantastic Arcana Note's style of item cards.

I couldn't find a session notes form or format that I liked, so I took some aspects from the character sheet PDF forms and made my own. I wanted to be able to hand write at the table and still be able to update it on my PC so the format handles linebreaks a bit oddly. It's very simple, but you're welcome to use/share it!
Here are some pdf form fillable spell cards that you can customize and print off. I've found them quite helpful for having a quick reference for your spell rules. It looks like I made sheets for Bard, Cleric, Druid, Paladin, Ranger, Sorcerer, Warlock, and Wizard.
