There are three keys. The key of terror, the key of hate, and the key of destruction. Terror drops from The Countess, Hate from The Summoner, and Destruction from Nithilak. When these three keys are combined together in a hoadric cube in act 5 hell you will open a portal to a boss.

Key drop rates for all three bosses first cap out at p7 when solo, or (if you're weird and run keys in groups) at p4 with all 4 players in the area together when you make the kill. Each boss has 5 'slots' in which they can drop a key, so each boss could technically drop 5 at once.
