2 lis 2018 · Applying for Retirement. A secure and enjoyable retirement is your reward for a career of public service. For a smooth transition, follow these steps at the recommended times. You should begin the process at least one year before your anticipated retirement date.
If you were hired by the State on or before June 30, 2011, and retired from State service on or before January 1, 2020, you are eligible to receive a State-funded HRA. Those hired on or after July 1, 2011, are not eligible for the State-funded HRA.
Use a pen and print clearly on the form. 2. Answer all questions in Section One accurately. 3. Sign and date the application. 4. Attach proof of birth date and make a copy of the form. 5. Submit the completed application to your retirement coordinator.
