Annual Dues of $12 should accompany the Application, made out to the General Society of the War of 1812. Please be sure to complete the portion showing the applicant’s name, address, telephone number, and email address, so contact can be made should a question concerning the Application arise
Each student should complete the application, gather additional required documentation, and submit everything to the Chariman National, 1812 Scholarship Committee no later than DECEMBER 31. Do not submit a photograph. For states and chapters, the application is located on the N.S.U.S.D 1812 member’s website.
