Procalcitonin has several advantages over CRP and ESR as a biological marker. Serum concentrations of PCT are normally <0.05 ng/mL. Procalcitonin levels become detectable within 3–4 hours and peak within 6–24 hours, which is earlier than both CRP and ESR.
1 maj 2019 · Since these same mediators drive the acute-phase response and elicit the increase in circulating CRP and fibrinogen (the major contributor to the ESR), the obvious question is why procalcitonin might be a more reliable biomarker to distinguish bacterial infection from inflammation or a viral infection than the CRP level or ESR.
Unlike CRP, which is a direct measure of inflammatory response, ESR is an indirect measure of the level of inflammation in the body. ESR measures the rate at which red blood cells settle in a specially designated tube of anticoagulated blood, an effect that is altered by proteins associated with an inflammatory response.
Our data demonstrate that WBC, but more CRP and PCT are reliable diagnostic and prognostic biomarkers, when considered in combination and with severity clinical score. PCT confirms its stronger usefulness as a diagnostic marker of sepsis.

Objective: A thorough understanding of the similarities and differences between these two serological markers, including factors that affect measurements, is necessary for the proper utilization and interpretation of ESR and CRP.
