3 sty 2012 · There are 12 cranial nerves, each with specific functions. They are numbered I through XII based on their position from the front to the back of the brain. Below is a chart of the 12 cranial nerves, the assessment technique used, if the response elicited is normal, and how to document it.
For the 2nd (optic) cranial nerve, visual acuity is tested using a Snellen chart for distance vision or a handheld chart for near vision; each eye is assessed individually, with the other eye covered.

Cranial nerve assessments help you deter-mine cerebral cortex and brainstem function depending on the patient’s level of conscious-ness (LOC). A fully awake and aware patient can follow commands that permit bilateral as-sessment of all 12 cranial nerves. A patient with a decreased LOC will have limited partic - ipation in a full assessment.
Visual acuity is formally assessed using a standard Snellen chart at a distance of 6m: Ask the patient to cover each eye in turn (you may need to do this for them) ask the patient to read the smallest line of letters they can.
