So in case anybody else can use it, I’ve collated a list of all COD Zombies maps... ever... and their respective DLC map pack. Edit: I’m keeping this list updated as new games and maps are released.

"Five" is the sixth Zombies map, and the second in Call of Duty: Black Ops. The map is unlocked after completing the final campaign mission, "Redemption", on any difficulty, or by entering the code '3ARC UNLOCK' on the computer terminal in the main menu, which will also unlock Dead Ops Arcade.
Pack-a-Punch has a unique set up on this map. In order to access Pack-a-Punch you must enable DEFCON 5. To do this you must locate the 4 DEFCON switches and press you activation button. The defcon switches are located in the War Room. Two are located on the top platform and two on the ground level. They do not be need to be activated in any order.
The map design is top tier and the pack a punch is the perfect level of difficulty and complexity to unlock. Riding a dragon to pack a punch is hard to beat. All of the side easter eggs on the map are so much fun to do and the shield is the favorite of any zombies map to date. Then there is the damn easter egg.
The DEFCON (Defense Readiness Condition) is a feature in the map that unlocks the Panic Room, the location of the Pack-A-Punch machine. To get the Pentagon to DEFCON 5 you will need to find four switches and take a teleporter.
