Abstract. The treatment-based classification (TBC) system for the treatment of patients with low back pain (LBP) has been in use by clinicians since 1995. This perspective article describes how the TBC was updated by maintaining its strengths, addressing its limitations, and incorporating recent research developments.

Time Based Classifications can be time based but even here there remains considerable variability and dis-agreement. There is no consensus on the length of time back pain must be present before it shifts from acute to chronic and no defined duration for the pain-free interval that distinguishes a new attack from a continuing chronic condition.
