Employers in California are required to establish, implement, and maintain written programs, depending on the hazards their employees are exposed to at work. To assist employers, Cal/OSHA has created many model programs and sample plans, which are templates that employers may download and customize to create their own written programs.
On the following pages, you’ll find: General instructions for filling out the forms in this package and definitions of terms you should use when you classify your cases as injuries or illnesses. An example to guide you in filling out the Log properly.
This is a fillable template that the employer must complete. Instructions in red font enclosed in brackets indicate where you must enter your worksite-specific information. Every California employer must establish, implement and maintain a written Injury and Illness Prevention
