10 mar 2023 · To submit electronically, create an SDI Online account at the EDD website. Select New Claim from the menu, and select Submit Electronic Paid Family Leave Care Attachment. Once EDD determines eligibility, intended parents can receive benefit payments for up to eight weeks in a year.
You can get a paper form at an SDI office, your employer, your doctor/medical practitioner's office, or by ordering a form online from EDD's Online Forms and Publications (the form will be mailed to you).
