A form, Claim for Paid Family Leave (PFL) Benefits - New Mother (DE 2501FP) will be sent to you after your pregnancy-related Disability Insurance claim ends. Note: Do not submit the DE 2501FP until after you have recovered from your pregnancy-related disability.
You will be paid if your employer pays employees on temporary disability, if you use accrued paid time off (such as vacation time), or if you apply to the California Employment Development Department (EDD) for State Disability Insurance (SDI) and qualify.

To submit electronically, create an SDI Online account at the EDD website. Select New Claim from the menu, and select Submit Electronic Paid Family Leave Care Attachment.
For faster processing, submit a claim form and documentation on SDI Online at www.edd.ca.gov/disability. To avoid delays when filing a claim using the paper application, please follow the general guidelines below. Allow sufficient time for claim processing (generally two weeks).
