Mothers without a pregnancy-related disability claim, new fathers, and foster or adoptive parents must complete sections one through five of the SDI Online bonding application, and provide a Proof of Relationship document. 10 mar 2023 · To submit electronically, create an SDI Online account at the EDD website. Select New Claim from the menu, and select Submit Electronic Paid Family Leave Care Attachment. Once EDD determines eligibility, intended parents can receive benefit payments for up to eight weeks in a year.
Complete and sign Part A on page 1 and Part B on page 3 of the Claim for Paid Family Leave (PFL) Benefits, DE 2501F, and mail no later than 41 days from the date you wish to begin your bonding claim. Attach documented evidence of your relationship to the child with whom you wish to bond.
You will be paid if your employer pays employees on temporary disability, if you use accrued paid time off (such as vacation time), or if you apply to the California Employment Development Department (EDD) for State Disability Insurance (SDI) and qualify.
