Infections can be asymptomatic or cause a malaria-like illness with fever and hemolytic anemia. Disease is most severe in asplenic patients, older patients, and patients with AIDS.
Symptoms of babesiosis usually start about 1 to 2 weeks after people are bitten. People may have fever, headache, muscle and joint aches, and fatigue. In healthy people, symptoms usually resolve after a week.
