5 mar 2024 · Left ventricular (LV) thrombus may develop after acute myocardial infarction (MI) and occurs most often with a large, anterior ST-elevation MI (STEMI). However, the use of reperfusion therapies, including percutaneous coronary intervention and fibrinolysis, has significantly reduced the risk.
The 2013 American College of Cardiology Foundation/AHA STEMI guidelines recommend as reasonable (Class 2a, Level of Evidence: C) 3 months of VKA therapy for patients with STEMI and asymptomatic LV mural thrombus with a target INR of 2.0-2.5 when combining OAC with DAPT. 5 The 2014 AHA/American Stroke Association stroke guidelines recommend 3 ...
The most likely explanations for a mural thrombus in the LV are severe LV dysfunction and/or recent anterior wall MI. Echocardiography is the primary modality to detect LVT.
The management of patients with left ventricular mural thrombus after AMI is challenging because insufficient evidence is available from randomized trials. In patients with or at high risk of developing LV thrombus, serial imaging may identify features warranting more intensive or prolonged antithrombotic therapy to reduce the risk of embolism.
Current treatment strategies for LV thrombus, including direct oral anticoagulant (DOAC) use, are also unclear and require examination . We identified patients with LV thrombus on echocardiography (with and without contrast) at Brigham and Women’s Hospital between January 2008 and May 2015.
14 wrz 2022 · We suggest that, on the basis of limited data, it may be prudent to treat patients with OAC for newly diagnosed mural (laminated) LV thrombus as one would a patient with a protruding or mobile thrombus.
