Alendronate is recommended as the first-line treatment; however, long-term adherence (both treatment compliance and persistence) is poor. Alternative bisphosphonates are available, which can be given intravenously and have been shown to improve long-term adherence.

Within the osteoclast, amino-bisphosphonates (eg, alendronate, risedronate, zoledronate) block the mevalonate pathway by inhibiting the enzyme farnesyl pyrophosphate synthase.
