  2. LiveWell, formerly MyAdvocateAurora, gives you access to your health record online and in our free mobile app. Message your doctor, view test results, start virtual visits and more.

  3. Learn how to use LiveWell with Advocate Aurora Health. LiveWell helps you manage the health and wellness of yourself and your family – it's an app and website that keeps you connected to your care, anytime and anywhere. You’ll be able to make appointments, send your doctor messages, check test results, pay your bill and more.

  4. From the LiveWell app, select MyChart, then Menu. Under Insurance, select Insurance Summary. Review, update, remove or add your coverage. As a reminder, please confirm Advocate Health Care or Aurora Health Care is in your insurance company’s network before seeking care with us.

  5. When you have a LiveWell account, you can manage health and wellness for you and your family – schedule appointments, message your doctor, check test results, pay your bill and more. Get answers to all your questions in our LiveWell FAQ.

  6. Patient Information. NOTE:You only need to use this form if you don't already have an account. Locked out of your account – Contact the LiveWell support team at (855) 624-9366 or email LiveWellSupport@aah.org.
