9 cze 2023 · Acetaminophen toxicity is the second most common cause of liver transplantation worldwide and the most common in the U.S. It is responsible for 56,000 emergency department visits, 2600 hospitalizations, and 500 deaths per year in the United States. Fifty percent of these are unintentional overdoses.
Because acetaminophen is ubiquitous and initially asymptomatic and treatable in overdose, consider toxicity in all possibly poisoned patients. Use the Rumack-Matthew nomogram when time of ingestion is known to predict risk of hepatotoxicity based on serum acetaminophen levels.
The use of paracetamol in children requires special care and maintain in an adequate dosage (based on age), which significantly differs from standard adult. The recommended dosage for children consider the metabolism of paracetamol, which determines the toxicity of the drug, especially hepatotoxicity (see below).

(2) Patients with polysubstance intoxication admitted to ICU for supportive care, who also happen to have a positive acetaminophen level.
