The Rumack–Matthew nomogram, also known as the acetaminophen nomogram, is an acetaminophen toxicity nomogram. It plots serum concentration of acetaminophen against the time since ingestion, in order to predict possible liver toxicity and allow a clinician to decide whether to proceed with N-Acetylcysteine (NAC) treatment.
8 paź 2021 · Consider acetaminophen toxicity in patients presenting with hepatic injury or failure. About half of cases are inadvertent, so there may be no obvious history of ingestion. When in doubt regarding the need to treat, it's safest to just give IV acetylcysteine (see tweet below).

The principal toxic metabolite of acetaminophen, N-acetyl-p-benzoquinone imine (NAPQI), is produced by the hepatic cytochrome P-450 enzyme system; glutathione stores in the liver detoxify this metabolite. An acute overdose depletes glutathione stores in the liver.
