I can't say with complete certainty and I have no idea how this was done while bypassing 2FA but it seems pretty clear that someone was able to hack my account and once inside they changed my password, signed out all devices and added their own to keep me locked out.

Number one reason why people have issues with account recovery is because other devices are still powered on. I'll explain further. When signed into iCloud on a device it will periodically send a "heartbeat" to Apple saying "hey, I'm here and active".
