On August 12, 1985, the Boeing 747 flying the route suffered a severe structural failure and decompression 12 minutes into the flight. After flying under minimal control for a further 32 minutes, it crashed in the area of Mount Takamagahara, 100 kilometres (62 mi; 54 nmi) from Tokyo.
On February 24, 1989, the Boeing 747-122 serving the flight experienced a cargo-door failure in flight shortly after leaving Honolulu. The resulting explosive decompression blew out several rows of seats, killing nine passengers. The aircraft returned to Honolulu and landed without further incident.

Both aircraft were destroyed by the impact and consequential fire and 583 people died. The Investigation attributed the crash primarily to the actions and inactions of the KLM Captain, who was the Operator's Chief Flying Instructor.
