The California Diabetes and Pregnancy Program recommends that women with values 140 mg/dl to 179 mg/dl should have a 3-hour 100 gram oral glucose tolerance test (OGTT) within one week. If the patient has a screening value > 180 mg/dl, a fasting blood sugar should be checked as soon as possible.
A blood sugar level of 140 mg/dL (7.8 mmol/L) to less than 190 mg/dL (10.6 mmol/L) signals the need for a three-hour glucose tolerance test to diagnose gestational diabetes. A blood sugar level of 190 mg/dL (10.6 mmol/L) or higher indicates gestational diabetes.
Results. In 7280 women with both GCT (135-199 mg/dL) and OGTT results, 4 GDM risk-equivalent GCT categories were identified with escalations at 144, 158, and 174 mg/dL (all p-values <0.05). The PPV for GDM increased from 33% to 64% as GCT increased from 135 to 199 mg/dL while the NPV decreased from 80% to 67%.

The ≥200 group had a higher incidence of preeclampsia (16.4% versus 10.6%) and shoulder dystocia (3.1% versus 1.0%).
