The tibialis posterior tendon often inserts with a broad attachment onto the ossicle, which may cause a painful tendinosis due traction between the ossicle and the navicular. Such changes are best seen on MRI.

Axial contrast-enhanced fat-suppressed MR image (TR/TE, 600/8) reveals excessive contrast enhancement around posterior tibialis tendon (arrow) in region of medial malleolus, consistent with synovitis. Enhancement is also seen in posterior tibialis tendon and is consistent with component of interstitial tear.
