1 paź 2019 · Drugs excreted by the kidney require dose reduction in chronic kidney disease. This adjustment depends on the severity of the disease and what proportion of the drug is eliminated by the kidneys. The estimated glomerular filtration rate can generally be used to guide dose adjustment in patients with stable kidney function.
16 lip 2021 · Kidney replacement therapy (KRT) adds a layer of complexity to drug dosing because the clearance of dialyzable medications differs significantly during the time on dialysis compared with when the patient is not undergoing dialysis, and drug clearance depends on any residual kidney function.
11 gru 2023 · This commentary provides an overview of recent recommendations concerning kidney function assessment that have important implications for drug dosing in patients with renal impairment and provides suggestions for implementing these recommendations.
27 lip 2024 · Renal disease primarily affects drug dosing in 3 ways: Patient susceptibility: Patients with renal impairment may be more vulnerable to a specific drug effect. Pharmacodynamic change: The effect of a drug may be increased or decreased in patients with renal disease.
Stepwise approach to adjust drug dosage regimens for patients with CKD and AKI. Abbreviations: AKI, acute kidney injury; CKD, chronic kidney disease; CL cr, creatinine clearance; eGFR, estimated GFR; GFR, glomerular filtration rate. Open table in a new tab.
Drug dosage adjustment for patients with acute or chronic kidney disease is an accepted standard of practice. The challenge is how to accurately estimate a patient’s kidney function in both acute and chronic kidney disease and determine the influence of renal replacement therapies on drug disposition.
