MyUCSDChart is a secure and portable electronic medical record that lets you access your health information online or on your mobile device. You can schedule appointments, view test results, message your providers, pay bills, and more with MyUCSDChart.

Log in to MyUCSDChart. Go to Menu (top left) and choose Medical Records (in My Records section). Choose Medical Record Request and follow the prompts. Your records will be delivered to your MyUCSDChart account, usually within three business days. Records that require a provider’s approval for release may take longer.
In order to grant you a MyUCSDChart account, we will verify your identity using questions from a third-party verification system. Once verified, you will be able to create your MyUCSDChart account.
