Normal CT appearance of the vermiform appendix except towards the tip. The distal end of the appendix appears abnormally swollen and edematous measuring 12 mm in caliber. It has no air loculi or contrast inside but is surrounded by perilumnal fat stranding.

28 wrz 2024 · CT. CT is highly sensitive (94-98%) and specific (up to 97%) for the diagnosis of acute appendicitis and allows for alternative causes of abdominal pain to be diagnosed. The need for contrast (IV, oral, or both) is debatable and varies from institution to institution.
Tip appendicitis describes a rare condition involving inflammatory changes of the distal appendix. We discuss the significance and management of this entity when it is identified on computed tomography (CT) imaging.
RESULTS: Of 18 patients with the CT finding of tip appendicitis, appendicitis was ultimately confirmed in 39%. Patients in this group had a higher rate of right lower quadrant (RLQ) tenderness (100% vs 55%, P .04), leukocytosis (14.2 vs 10.5, P .03), and clinical suspicion for acute appendicitis (43% vs 0%, P .02).
The CT finding of tip appendicitis can be managed conservatively in a subset of patients with low clinical suspicion for acute appendicitis.
Tip appendicitis is an uncommon variant of acute appendicitis. It is very important to assess the appendix along its course, the base could be normal with a diseased tip and vice versa. This is a little bit difficult in ultrasound however it is slightly easier in CT.
