Melanoma arises in up to 15% of children 10 years or younger with a giant congenital melanocytic naevus >40 cm in diameter (G3) particularly if associated with neurocutaneous melanocytosis. Such giant congenital melanocytic naevi are very rare, arising in 1 in 20,000 births.
