I've heard this issue is caused by people wrapping the wire around the controller for storage, causing the bit near the base to bend. try the controller on another port (with a 2 player game maybe), maybe the port has a bad connection?

Bluetooth connection cord checked on the board if it is properly pint to the board. Sounds like a break in the cable somewhere. The wires inside the cable between the controller and Xbox port are most likely broken right after the strain relief by the controller.
Could be some thing as simple as the board being dirty or a short in the cord/controller port. Have you tried multiple ports? Yes in the 4 slots, I have another controller that still works and it doesn't give me any problems in the tester.
