This trinket is a reference to religious tracts. Its sprite also resembles a Chick tract, a brand of comic-style tracts. This is further supported by Chick tract style comics being included in some physical editions of The Binding of Isaac.

Monkey Paw - When you reach half a heart of life remaining, a black heart will spawn. After three black hearts have spawned, the trinket dissapears.
According to the rebirth wiki (not platinumgod), the Bible Tract can make angel rooms appear after you've taken a devil deal. I just finished floor 50 on my current BASEMENT run with the bible tract and still have not run into an angel room. Just curious if it actually does something.
