To connect a wireless controller to your smart TV: In the Xbox app, use your TV’s remote control to select the Controller icon at the top of the screen. Select Pair a controller and then choose your controller from the selection list. Put your controller into pairing mode.
If you're using wireless on PC, you'll need the Xbox Wireless Adapter if your PC doesn't already support a compatible connection. If you don't want to use batteries at all you can just use a USB-C cable and it will be a wired controller.
TL;DR: USB 3.0 interferes with wireless devices. I sat a usb 3.0 SSD on top of my console that was causing my controller to disconnect. Got a longer cable for my SSD, move it as far away from the xbox as I could. Going on 2 weeks, longest I've had without any disconnections.
Note: if you see this behavior, try reconnecting the controller to the console via USB/Wireless or if possible connect another controller and submit feedback. Then reboot the console and try re-syncing the controller.
