The reason this happens is because you've tried to charge your controller play and charge kit with a power brick with a high wattage. Anything above 5 watts will eventually damage the chip and it will no longer charge correctly.
I connect the leads and trickle charge dead battery packs for 5-15 minutes on a 5 volt 1 amp power supply to see if cells are actually bad, or its just dropped below the threshold.
