I've been using keypad controllers for over a decade. Right now I've got the Razer Orbweaver; I could never go back to a standard keyboard. Use that along with a 6-button mouse and program buttons 5 and 6 for alt/shift modifiers.
I have a key bound to 1 as the primary and then alt-1 as the secondary. I have set the actuation points to the minimum and maximum values. So when I lightly press the button, it inputs 1 and then when I press the button all the way down it inputs alt-1.

In the past it was necessary to use an external program, such as xpadder, Steam, or the ConsolePort addon’s WoWmapper companion, to control the game with a gamepad such as the Xbox, Playstation, or Steam Controller.
