10 maj 2018 · The MyUHealthChart patient portal acts as a door to your medical chart, allowing fast access to most of your current UHealth medical information, with occasional exceptions. A biopsy report, for example, is something your doctor would discuss with you in person.
Enter your activation code as it appears on your enrollment letter or After Visit Summary®. Your code is not case sensitive. Please provide email submitted during registration. If you do not remember submitting an email address during your visit, please contact us at 305-243-4551.
