Download the latest version of the application form here. You can also accomplish the form by hand. The application form must be printed on A4-sized paper. You can accomplish the form using either the English or Korean language. Write in all capital letters when filling out the form.

The sponsor (inviter) must download Visa application form (for the applicant) from “Application Status” on the Visa Portal, and send it to the applicant via email, or inform him/her of the number and expiry date of the confirmation of visa issuance.
