Docket is a new app from the NJ Department of Health that makes it easier for you and your family to securely access your COVID-19 vaccination record. If you don't have your vaccination card with you or you have lost it, you can easily access your record through the app to provide proof of COVID-19 immunizations as needed.
The COVID-19 vaccine helps protect you from severe disease, hospitalization, and death. It is especially important to get your 2024–2025 COVID-19 vaccine if you are ages 65 and older, are at high risk for severe COVID-19, or have never received a COVID-19 vaccine.
Alternatively, you can request an official copy from your state immunization registry. New Jersey residents vaccinated in-state can request a copy through NJIIS or use Docket (Refer to question 4 below).
