Medical Information. The Boy Scouts of America recommends that all youth and adult members have annual medical evaluations by a certified and licensed health-care provider.
This Annual Health and Medical Record is valid for 12 calendar months. ADULTS AUTHORIZED TO TAKE YOUTH TO AND FROM EVENTS: You must designate at least one adult.
