To obtain a health card, visit the nearest PHCC’s health center to your residence. There you need to fill out the appropriate forms. You will be issued a PHCC health file number. Please take with you: Qatari Adults: A valid Qatar Identification Card (QID), 4cmx3cm photo, and credit/debit card to pay the 50 QR fees.
