From someone in a real military I'm always confused about how starfleet chooses who's on the bridge and what rank has to do with anything. Ensigns as ops Bridge officers? Lieutenants in charge of major departments like engineering?
By a conservative estimate, i'd say that amounts to roughly half of the crew (all people "not in uniform / not within the military rank structure", including spouses, children, civilian scientists, civilian staff etc.)
I think that the rank structure in Starfleet is flatter than it appears. Role appears more relevant than rank (for example, non-com Chief O'Brien commanding officers on DS9's engineering team).

In later television series, ranks are indicated by varying numbers of pips or bars on the individuals' uniform collars. A "pip" refers to a decoration worn on a military uniform to denote rank, such as on British Army officer rank insignia.
