16 wrz 2024 · Medial sphenoid wing meningiomas are best treated through pterional craniotomy, as pterional craniotomy provides wide and multidirectional exposure of the anterior and middle cranial fossa. Anterior clinoidectomy can increase the exposure potential.
Resecting MSWMs through the classical pterional approach can be technically challenging, in large and giant lesions. In the extended pterional approach, the frontal bone cut is performed touching the ipsilateral supra orbital prominence but not the orbital rim.
Depending on the degree of mass effect caused by the tumor, mannitol may be administered prior to the craniotomy. Dexamethasone is used in patients with significant vasogenic edema. I use embolization sparingly in patients with an AVMs or meningiomas. Neuronavigation is usually used.
1 wrz 2023 · The pterional approach and anterior interhemispheric approach are two possible surgical options for the treatment of anterior skull base meningiomas. Both approaches have achieved optimal results, with total tumor resection in most patients and a low complication rate, and that the best approach depends mainly on the experience of the surgeons.
7 kwi 2023 · Meningiomas involving both the anterior and middle skull bases, such as the sphenoid wing meningioma and spheno-orbital meningioma, require exposure to both the frontal and temporal areas. In these cases, a full frontotemporal or pterional craniotomy is warranted.
The extended pterional approach can replace bifrontal craniotomy for treating giant midline anterior cranial fossa intradural lesions (ie, olfactory and cribriform plate meningiomas; Figure 2). 8, 9 With the exception of extradural skull base tumors that extend into the intradural space (ie, esthesioneuroblastomas, chondrosarcomas, and other ...
The pterional, or frontotemporal, approach was developed by Gazi Yasargil in the 1970s. [33,34] It is the most widely used approach for supratentorial lesions. This approach is the standard craniotomy for treating anterior Circle of Willis aneurysms and the cavernous sinus.
