I've watched YouTube videos and read posts but I can't get the Altar crafting process to work. I select a recipe and then click on the four squares in the Crafting window and select a beast for each one.
The Blood Altar can be found in various locations throughout Path of Exile. But you can most commonly find the altar in Incursion rooms or the Temple of Atzoatl. They can also appear in other areas, but these are the ones where you can most reliably find them. Gather the Required Sacrifices.
