Patients can request electronic medical records for personal use through MyChart. After entering the required information, the records are released through MyChart at no charge. Simply log into the MyChart account and follow the links under Menu, Medical Records, Medical Records Request.

Your data is stored with your healthcare organization. Epic does not have access to your data and cannot reset your passwords for you.
