Complete and return your PL Proposal Form. Selected proposals will work with the ROE 48 PL Team to develop your offering. You will then earn a percentage of the enrollment fees generated by your PL.
Download and complete the Work Permit Application (PDF), filled out by both the student and employer. School Physical or copy of Sports Physical from within the last year. Copy of the student's birth certificate. Note: The student must be 14 or 15 years old.
This completed form shall serve as notice to any school district, Regional Office of Education, or truant officer that the indicated parties have registered with Peoria County ROE #48 as homeschool students.
