Pacemakers can be programmed to pace and to sense in a bipolar mode (lead tip to lead ring) or a unipolar mode (lead tip to pulse generator). Bipolar sensing is preferred due to its smaller sensing “antenna,” which minimizes oversensing of extracardiac signals.

A VVI pacemaker stimulates and senses in the chamber and if it senses spontaneous ventricular activity (R-wave), it does not stimulate. If the ventricular rate is slower than the base rate, then the pacemaker will pace.

Comparing VVI and VVIR modes, grouped together and individually, with DDI pacing modes showed no significant differences in all cause mortality. At three years heart failure, myocardial infarction, new onset angina, and pacing system revision were similar in all groups.
